What the Record Shows
How a 1949 city hospital became a university asset
The City of Tallahassee founded the hospital in 1949 and moved it to a lease-based model in 1979, with a nonprofit operator — today’s TMH — running it under a $1-per-year lease while the City retained ownership of the land, the buildings, and the equipment down to the hospital beds and X-ray machines. TMH grew into a 21-county regional system with a 17-member board.
The FSU relationship deepened after the Legislature and Governor funded an academic health center at TMH in 2021. FSU broke ground in 2023 on the FSU Health Research Center, a five-story facility on the northeast corner of the TMH campus off Centerville Road, backed by $125 million in state money and scheduled to open in late 2026. In September 2025, TMH staff learned by internal email that the hospital and FSU had struck a “landmark agreement” for TMH to become part of FSU Health.
The vote, the deadline, and the gap
City staff flagged in January 2026 that TMH’s expansion beyond Tallahassee and its request to reshape governance might require a change in the lease structure and a review of whether continued City ownership served the City’s long-term interest. The Commission accepted a Memorandum of Understanding with FSU on January 14 and, after public pushback, approved the asset transfer 3–2 on March 11. FSU’s trustees and the Board of Governors approved on March 26, and the Board of Governors extended President Richard McCullough’s contract to 2029 at the same meeting.
The MOU established that an FSU Health board would be created only if TMH and FSU signed a new lease. That lease was not signed by April 1. At transfer on April 10, FSU’s release confirmed TMH continues to run the hospital, described it as the “clinical anchor” of FSU Health, and did not announce a lease. Commissioner Jack Porter’s warning from before the vote captures the stakes: without an FSU–TMH agreement, the City has transferred the asset without a guarantee that an academic medical center results.
Both institutions have privately said the deal will get done. The proposed lease reportedly gives TMH nine board seats to FSU’s eight, keeps politically appointed FSU trustees off the board initially, and gives TMH right of first refusal if FSU ever sells. None of that is binding until executed.

FSU’s motivation is academic capacity. The university had 1,772 nursing students in 2025 who need clinical hours, and a College of Medicine — the first new U.S. medical school in 20 years when the Legislature created it in 2000 — that has never controlled a teaching hospital. The transfer opens specialty rotations in pediatrics and emergency medicine and new residency and fellowship programs in cardiology and pulmonary care.
In exchange, FSU takes on $109 million in purchase payments, $250 million in committed investment through 2034, a perpetual deed restriction to operate the property as a hospital, and the City’s charity-care commitments. That is a $359 million minimum obligation before a single new building beyond the Research Center is funded.

I’ve watched enough transactions to know that “privately, both sides say it will get done” is usually true and occasionally famous last words. The 9–8 board split, the exclusion of political appointees, and TMH’s right of first refusal are all reasonable terms — they’re also exactly the kind of governance details that stall when the lawyers on each side count votes. Until the lease is executed, FSU has paid for a landlord’s position and TMH has a landlord with a $250 million checkbook and no board seat. That is a stable arrangement for a while, and it is not an academic health center. The Research Center opens this fall.
